Free printable attendance sheet template
Download a free attendance sheet template as a PDF, in US Letter or A4. It has a header for the event, date, location, and facilitator, and numbered rows with columns for full name, organization or group, time in, and signature. Print it and you're ready — no sign-up needed.
What's on the template
Portrait, 24+ numbered rows per page, with these columns:
How to use it
- Download and print. Pick US Letter or A4 and print as many pages as you expect attendees, plus one spare.
- Fill in the header. Write the event or class name, date, location, and facilitator before anyone signs.
- Collect sign-ins. Place the sheet on a clipboard at the entrance and ask each person to print their name clearly, note their group, the time, and sign.
- Store or digitize. Keep the signed sheets with your records, or type them into a spreadsheet if you need a digital copy.
Tips for cleaner records
- Ask people to PRINT their names — signatures alone are rarely legible.
- Number your pages ("page 1 of 3") so a missing sheet is obvious.
- Write the start time in the header so late arrivals are easy to spot from the Time In column.
- Keep sheets with personal data somewhere secure, and shred them when you no longer need them.

Paper works, but someone has to read the handwriting and type it up afterwards. With AttendPad, people scan a QR code and sign in on their own phone. You see the list live, late arrivals are flagged, and it exports to CSV in one click. No app to install, and the free plan includes 3 active sheets and 1 room.
Frequently asked questions
What should an attendance sheet include?
At minimum: the event or class name, the date, and each attendee's name. Most attendance sheets also include a time-in column and a signature column as proof of attendance, plus an organization or group column for meetings and trainings.
Is this attendance sheet template free?
Yes. Download and print it as often as you like, with no sign-up or email required.
Can I use this template for a class or meeting?
Yes. It works for classes, meetings, trainings, workshops, and events. Write the session name in the Event / Class field.
Do you have an Excel or Google Sheets version?
Not yet. If you want attendance that's digital from the start, an AttendPad sheet exports straight to CSV, which opens in Excel and Google Sheets.
What's the alternative to a paper attendance sheet?
A digital attendance sheet: attendees scan a QR code with their phone and sign in, and you get a live list you can export. AttendPad's free plan includes 3 active sheets.
