Class Attendance Sheet Template for Gyms & Studios
A studio sign-in sheet is doing two jobs at once: knowing who is in the room for capacity and safety, and having a signature on file that says the participant accepted the risk.
Here's a printable version, and how the same check-in works when the clipboard by the door becomes a scan.
No sign-up needed to print. Free tier for the digital version — no credit card.

Capacity, liability, and the no-show problem
Class capacity is the constraint that makes attendance operational rather than administrative. If a studio holds sixteen and eighteen people arrive, someone has to know that before the class starts, not from a clipboard someone counts afterwards.
Liability is the second driver. A signature acknowledging a waiver has to be attached to a specific person on a specific date, which is exactly the thing a scanned paper sheet is worst at proving.
The third is commercial: no-shows in booked classes cost you a paying slot. Consistent attendance records tell you which classes and which times actually fill, and which members are drifting before they cancel.
The same routine, without the transcribing
Print the sheet above and it works. What it can't do is add itself up. If you're doing this every day, here's what changes when the sheet becomes a QR code on the wall:
Live headcount against capacity
Set a maximum on the room and watch the count as people scan in. You know you're at fifteen of sixteen before the class starts rather than after.
Waiver signature captured at check-in
Collect a digital signature as part of the check-in itself, tied to that person and timestamp. It exports alongside the attendance record rather than living in a folder.
Check-out for time-based access
For open-gym or day-pass access, scanning out records actual duration — useful for occupancy reporting and for spotting who is using a membership heavily enough to upsell.
Per-class and per-trainer records
Each class or trainer can have its own room, so attendance by session and by instructor comes out of the export without you sorting anything by hand.
Recommended setup
Use a Room per class or per space, with capacity set and signature capture on. If you run day passes or open gym, turn on check-out so you get duration rather than just arrivals.

Frequently asked questions
Can I cap the number of people who check in to a class?
Yes. Set a maximum capacity on the room and the live count shows against it as people scan in, so you can see you're approaching the limit before the session starts.
Can members sign a liability waiver when they check in?
Yes. Enable signature capture at check-in and the signature is stored against that person and timestamp, then included in the export. You can make it required so check-in can't complete without it.
Can I track how long someone was in the gym?
Use a room with check-out enabled. Members scan the same QR code on the way in and on the way out, and total duration is calculated automatically. Automatic checkout closes out anyone who forgets.
Do members need to install an app?
No. They scan the QR code with their phone camera and the check-in form opens in the browser. No app, no account. A tablet in kiosk mode at the front desk covers anyone without a phone.
