Church Attendance Sheet Template
Church attendance covers three quite different situations under one heading: a service where the priority is welcoming visitors, a small group where the priority is continuity, and children's ministry where the priority is knowing exactly which adult may collect which child.
Below is a printable template plus how each of the three works digitally.
No sign-up needed to print. Free tier for the digital version — no credit card.

Three different jobs, three different sheets
For a service, the record that matters is the visitor. Regulars don't need signing in; someone attending for the first time does, because a name and a contact detail captured at the door is the entire basis of follow-up that week. A sheet designed for roll call gets this backwards.
For small groups, the value is in the pattern over months. Who has quietly stopped coming is pastoral information, and it's invisible unless attendance is recorded consistently.
Children's ministry is the one with real safeguarding weight. The check-in record has to establish who dropped a child off and who is authorised to collect them, and it needs to hold up under scrutiny rather than being a name in a column.
The same routine, without the transcribing
Print the sheet above and it works. What it can't do is add itself up. If you're doing this every day, here's what changes when the sheet becomes a QR code on the wall:
Visitor-first check-in for services
A short form at the door — name, contact, first time here? — with a QR code on the welcome desk. Custom fields cover ministry interest or how they heard about you.
Child check-in and pickup
Record who dropped the child off and capture a signature at collection, so both ends of the handover are on file with timestamps rather than resting on recognition.
One code per group, week after week
A small group is a room: one permanent QR code, a record for each meeting, and months of history without creating a new sheet every week.
Records without the Sunday admin
No transcribing clipboard sheets on Monday. Export the range you need for a board or denominational report straight to CSV or PDF.
Recommended setup
A Room for each recurring gathering — the service, each small group, each kids' class — and Sheets for one-off events like a conference or a workday.
More detail on the mechanics: QR code attendance, daily check-in rooms, and what's free vs Pro.
Frequently asked questions
Can I tell visitors apart from regular attendees?
Add a custom field — a first-time-here checkbox, or a member/visitor choice — and it comes through on the live view and in the export, so the follow-up list is filterable rather than something you reconstruct.
Can I use this for children's ministry check-in and pickup?
Yes. Use a room with check-in and check-out signatures enabled: a signature at drop-off and one at collection, each timestamped. Note that both ends of that handover are still a human process — the record supports your safeguarding policy, it doesn't replace it.
Do we need a new sheet every Sunday?
No. A room keeps one permanent QR code and creates its own record for each day, so the same printed code on the welcome desk works indefinitely.
Is there a free option for a small congregation?
The free tier covers one room and three active sheets with unlimited check-ins and CSV export, which is enough for a single service or one small group. Multiple groups or kiosk mode need Pro at $9/month.
